ABOUT TRANSFORMATIONS TREATMENT CENTER
Located five miles from the coast in Delray Beach, in the recovery hotspot of South Florida, Transformations Treatment Center offers outpatient treatment with sober-living facilities, to provide a residential program in the Florida Model. Medical detox is not available on-site, but Transformations partners with a facility in nearby Boynton Beach to provide detox services.
The center accepts and treats dually diagnosed clients, and offers dedicated treatment tracks for young adults and veterans (who'll find specialized services at 29.2 percent and 25.9 percent of Florida-based facilities, respectively), as well as first responders and Christians. Transformations is among the 52.7 percent of rehabs in Florida that operate as private for-profit companies.
TREATMENT & ASSESSMENT
Transformations offers a partial hospitalization program (PHP), offered at 24.6 percent of Florida-based facilities, and an intensive outpatient program (IOP), offered at 43 percent of Florida-based facilities, as well as standard outpatient care and aftercare. The length of treatment varies according to the client's needs, but is typically between one and six months.
Treatment is based on the 12-Steps, supplemented by group and individual counseling. When appropriate, clients struggling with opioid addiction may received medication-assisted treatment (MAT) using Suboxone. And for those clients who have experienced trauma, Transformations offers eye movement desensitization and reprocessing (EMDR).
Clinical services are supplemented by holistic provisions such as a vitamin and supplementation program, to ensure wellness, as well as music therapy with an on-site studio and adventure therapy that takes clients out into the natural environment of Florida, exploring the waterways and other natural attractions.
The treatment team at Transformations Drug & Alcohol Treatment Center includes social workers, licensed mental health counselors, certified addiction professionals and masters-level therapists. The 11 individuals polled by Rehabs.com to date on the staff gave positive feedback.

While they complete outpatient care at Transformations, clients live in one of two housing options: Via Delray, a block of 36 condos, each with two bedrooms, or the Villa, a three-level home with six bedrooms for two people each. Both centers offer sundecks, ping-pong tables, communal indoor and outdoor space, and full kitchens. Alumni polled by Rehabs.com gave mostly positive feedback, with some notes of strong complaint. The seven alumni polled on the facility's offerings in a variety of metrics indicated that they would recommend the center and were impressed by its family programming and holistic options.

The reviewers repeatedly praised the center's caring staff as well as its aftercare. "Transformations literally transformed my life. In am so blessed to have found this place. The staff at the clinical building are outstanding and genuinely care," Jessica wrote in one representative comment. "Inpatient care was great but the aftercare was even better! They are there for you every step of the way as soon as you step through their doors," Amy added in another. Numerous reviewers also reported that they had successfully got sober at Transformations after failed attempts elsewhere, and some even credited the facility with saving their lives.
However, more than one reviewer complained of both dishonesty and unhelpful staff: "Severly misinformed me just to get me there...With the exception of a few techs, 1 physicians assistant and one M.D., staff had zero compassion and most were very unprofessional," S.P.M. wrote in a typical negative review.
Secondary sites offer a similar pattern of mostly positive feedback, punctuated by some notes of strong complaint. The facility has average ratings of 4.2 out of five stars based on 64 reviews on Google, 4.6 stars based on 154 opinions on Facebook (where the center can managed its own page), as well as many five-star ratings on Yelp and CiteHealth, most of them "not recommended" by Yelp's authenticity algorithm.   
As with reviews submitted to Rehabs.com, many alumni offered glowing praise and thanked Transformations for saving their lives; however, others claimed the facility was more interested in making money than helping people, and complained of questionable intake and contract practices.

Loved ones polled by Rehabs.com also gave mostly positive feedback with some notes of complaint. The four individuals polled on the facility's performance in a variety of areas indicated that they were impressed by the center's family participation, but less so with its counseling options:

Parents repeatedly expressed their relief at having found a center that seemed to be helping their child. "My daughter received really great care during the 4 months she was in therapy at Transformations... The therapists were insightful and responsive, and the staff really cared about her," Gina wrote in a representative comment. However, the reviews also contain complaints about ineffective treatment and poor communication.
WHAT STAFF SAY
The four former staff members polled by Rehabs.com to date gave polarized reviews, with two staff members giving the facility excellent ratings and two giving the facility poor ratings. Two of the four reviewers reported they would recommend the facility, while three staff members felt leadership of the facility was willing to put clients' interests first.
When asked about the strengths of the facility, staff members described caring staff and good discharge and aftercare planning. One staff member wrote, “Facility cleanliness is impeccable. Client to Therapist ratio is small. Clients have many opportunities to customize their treatment plan to fit what they want.”
When asked about the weaknesses of Transformations, staff reported that the decision-making was inconsistent at the clinical level, management was interested primarily in money, and that the facility was new and less experienced than some other facilities.
Transformations doesn't publish the cost of its programs, but does accept insurance on an out-of-network basis. The 10 individuals polled by Rehabs.com to date on the center's affordability gave mixed feedback:
